A systematic review of postmenopausal women with osteoporosis or low bone mass was conducted to assess the impact of exercise on fall-related functional performance. The review aimed to determine whether intervention effects differed according to the reported consistency of the exercise prescription with American College of Sports Medicine recommendations.\\ \ The systematic review searched six electronic databases from inception to 3 May 2026. The review identified 18 trials contributing 21 intervention arms, with results indicating that exercise improved Timed Up and Go performance and 30-second sit-to-stand test performance. However, the estimated effect on Berg Balance Scale performance was highly uncertain. \\ \ Formal tests found no evidence of a difference between the high and low/unclear reported-consistency groups for Timed Up and Go, Berg Balance Scale, or 30-second sit-to-stand test. Prediction intervals crossed the null for all three outcomes. The certainty of evidence was low for Timed Up and Go and 30-second sit-to-stand test, and very low for Berg Balance Scale. \\ \ The review suggests that exercise may improve functional mobility and repeated sit-to-stand performance, while the effect on Berg Balance Scale remains uncertain. The subgroup findings, which explored the impact of exercise prescription consistency with American College of Sports Medicine recommendations, were exploratory and did not demonstrate superior effects. \\ \ \
