Raynaud's phenomenon is an early manifestation of systemic sclerosis, reflecting underlying microvascular dysfunction. Exercise improves vascular function, but the magnitude and nature of microvascular impairment relative to healthy physiology remain poorly characterised. This study compared resting digital endothelial-dependent and endothelial-independent microvascular responses, together with systemic microcirculatory responses to exercise, between people with limited cutaneous systemic sclerosis and age- and gender-matched healthy adults.
Microvascular function was evaluated using laser Doppler flowmetry with iontophoresis of acetylcholine and sodium nitroprusside. Systemic physiological responses to exercise were assessed using transcutaneous oxygen pressure measured from the scapular region during moderate-intensity constant-load exercise.
Significant baseline differences were observed for acetylcholine responses between people with limited cutaneous systemic sclerosis and healthy adults.
The study's findings suggest that microvascular dysfunction is a characteristic feature of limited cutaneous systemic sclerosis, with implications for understanding the disease's pathophysiology.
However, the study's results should be interpreted with caution, as the sample size is relatively small and the study design is cross-sectional.
