Predictive Value of Uterine Blood Flow Parameters in PCOS Patients Undergoing IVF

Researchers conducted a retrospective study to evaluate the predictive value of endometrial and uterine artery blood flow parameters, combined with clinical indicators, for embryo implantation outcomes in patients with polycystic ovary syndrome (PCOS) undergoing in vitro fertilization-embryo transfer (IVF-ET). Understanding these factors is relevant for metabolic research contexts where optimizing reproductive outcomes through physiological markers remains a key area of investigation.

The study analyzed 129 PCOS patients who underwent IVF-ET at a single hospital from June 2022 to June 2025. Participants were stratified into four BMI groups, and transvaginal ultrasound parameters alongside clinical data were statistically analyzed to assess correlations between blood flow dynamics and implantation success.

Significant differences were observed in endometrial thickness, morphological patterns, blood flow distribution, and uterine artery blood flow parameters across different BMI groups. The obese group exhibited a tendency toward thinner endometrium, a lower proportion of type A endometrial morphology, reduced endometrial perfusion, and a higher uterine artery resistance index. Logistic regression analysis identified high endometrial blood flow grade, abundant endometrial blood flow, and a greater number of high-quality embryos as protective factors for successful embryo implantation following IVF-ET in patients with PCOS.

These findings suggest that blood flow parameters may serve as potential biomarkers for predicting implantation success in this population. However, the study is limited by its retrospective design and single-center nature, which may affect generalizability. Results should be interpreted within the context of research use only, without clinical recommendations or medical advice.
